Myasthenia gravis and muscles involved

Today, in the study group, someone asked about nerves not involved in MG.

Myasthenia gravis can present as a pupil-sparing third nerve palsy with ptosis.

Why is the pupil spared?

Because MG does not affect involuntary muscles such as the heart, smooth muscles of the gut, blood vessels, and uterus. Pupillary muscles are also involuntary smooth muscles and hence they are spared.
