Platypnea-Orthodeoxia Syndrome in interatrial right-to-left shunt

Hi everyone! This is a short post :)

Platypnea (flat breathing): Dyspnea induced by upright posture and relieved by recumbency.

Orthodeoxia: Arterial oxygen desaturation accentuated by upright posture and improved by recumbency.


Etiology: Interatrial right-to-left shunt through a patent foramen ovale, or an atrial septal defect or an atrial septal aneurysm.

Pathophysiology: Incompletely understood. Standing upright could stretch the interatrial communication, be it a patent foramen ovale, an atrial septal defect, or a fenestrated atrial septal aneurysm, thus allowing more streaming of venous blood from inferior vena cava through the defect.

Treatment: percutaneous transcatheter closure of interatrial shunting.
