It is used for holding stuff.. Holding lens during its placement, holding the anterior capsular flap and (then tearing it out) during ECCE and so on.
I have no idea why there are three holes in the handle.. Probably to make to easier for our fingers to hold. (Lemme know in the comments if you know the reason.)
